» 2026 Domestic Use Video License Agreement form – required if you wish to record a video of your family member’s performance. Enquiries to 02 9935 7707. Where the form asks for an ABN number, put n/a if you don’t have an ABN.

» ​Novice Eligibility Verification (for Instrumental & Piano Solo only – note criteria – 18 months or less learning 1st instrument)

» Performers’ ​Copyright Declaration Form (specifically for Balaklava Eisteddfod – to be completed by all entrants in music sections​

* Please note only ONE Performer Declaration form is needed per School / Entrant for all sections. The form is to be kept with the entrants and taken to the booking in office of each venue.

Friday Entries

  • TOTAL ENTRIES are limited to a maximum of FIVE per school.
  • No more than TWO entries per Band, Choral and Rock Band discipline are allowed for Friday. ONLY ONE SENIOR Rock Band may be entered due to the popularity of that section.
  • MORE than two entries are allowed in Instrumental and Vocal Ensemble disciplines, providing the total number of entries per school does not exceed five.

Saturday Entries

  • TOTAL ENTRIES are limited to a maximum of FIVE per individual for ALL Saturday disciplines.
  • EACH SOLO ENTRANT MUST complete an entry form & pay $20 per solo entry fee.
  • EACH MEMBER of an ensemble or group (2-5 performers) MUST complete an entry form & pay a $10 entry fee.
  • An entrant may enter more than one ensemble or group in a discipline, providing the total number of entries does not exceed five.
