» Novice Eligibility Verification (for Instrumental & Piano Solo only – note change in criteria – now 18 months or less learning 1st instrument)
» Performers’ Copyright Declaration Form (specifically for Balaklava Eisteddfod – to be completed by all entrants in sections other than Speech & Drama Discipline)
* Please note only ONE Performer Declaration form is needed per School / Entrant for all sections other than Speech & Drama, which uses a different copyright declaration form. The form is to be kept with the entrants and taken to the booking in office of each venue.
