» ​Novice Eligibility Verification (for Instrumental & Piano Solo only – note change in criteria – now 18 months or less learning 1st instrument)

» Performers’ ​Copyright Declaration Form (specifically for Balaklava Eisteddfod – to be completed by all entrants in sections other than Speech & Drama Discipline)​

* Please note only ONE Performer Declaration form is needed per School / Entrant for all sections other than Speech & Drama, which uses a different copyright declaration form. The form is to be kept with the entrants and taken to the booking in office of each venue.

Friday Entries

  • TOTAL ENTRIES are limited to a maximum of FIVE per school.
  • No more than TWO entries per discipline are allowed for Friday.

(for Band (Concert & Stage), Instrumental Ensemble, Rock Band, Choral & Vocal Ensemble. One entry fee per group)

Saturday Entries

  • TOTAL ENTRIES are limited to a maximum of FIVE per individual for ALL Saturday disciplines.
  • EACH MEMBER of a small ensemble or group (2-4 performers) MUST complete an entry form.
  • ONLY ONE form to be completed for large group drama (5-9 performers).
